To begin, please complete the intake form below. Once we receive it, we will start the intake process.
Referral
As we work together to complete your intake, please reach out to your child’s primary care doctor and request a referral/order for the service(s) selected. Your doctor can email or fax the referral to our office at 405. 883. 3061. We REQUIRE a referral/order for all services billed through insurance.
Benefits
During this step, we will discuss benefits together and provide an estimate for services. Depending on your payment selection and benefit requirements we may be required to obtain authorization for the evaluation which can take a minimum of 5 business days depending on insurance and clinician availability.
Evaluation
It’s evaluation time! Your child’s evaluating clinician will be in touch with you before the evaluation to answer any of your burning questions. Evaluations vary in length, but in general plan at minimum of one hour for this appointment.
Consultation
The consultation appointment will be 5-30 business days following the evaluation. This appointment will go over the evaluation results. This is also when we will begin therapy (if warranted) and set up recurring appointments. We believe in family-first therapy, and try our best to accommodate families’ schedules to the best of our ability!
