Getting Started is as easy as ...

Intake

To begin, please complete the intake form below. We know the form is not short, but it is comprehensive, we ask that you take your time and fill it out to the best of your ability. Forms that have missing information or not enough detail require additional time to track down the answers we may need to get started!

Referral

As we review your intake form, please reach out to your child’s primary care doctor and request a referral/order for the service(s) selected. Your doctor can email or fax the referral to our office at 405. 883. 3061. We REQUIRE a referral/order for all services billed through insurance.

Benefits

Private Pay clients skip this step! During step 3, we will contact your family’s insurance(s) and obtain benefit information. We HIGHLY encourage you to do the same. We will learn information about eligibility, benefits, and prior authorization requirements. Once this is completed you will receive a follow-up email with an estimate and explanation of benefits for your signature.

Evaluation

It’s evaluation time! Your child’s evaluating clinician will be in touch with you before the evaluation to answer any of your burning questions. Evaluations vary in length, but in general plan at minimum of one hour for this appointment. Following the evaluation, your clinician will write the evaluation report and submit it for prior authorization if necessary.

Consultation

The consultation appointment will be 1-4 weeks following the evaluation. This appointment is 25-30 minutes long and will go over the evaluation results. This is also when we will begin therapy (if warranted) and set up recurring appointments. We believe in family-first therapy, and try our best to accommodate families’ schedules to the best of our ability!
